西新井内科クリニック院長の森田知宏（もりた・ともひろ）は、東京大学医学部を卒業後、亀田総合病院で初期研修を行い、福島県相馬市の相馬中央病院で地域医療に従事してきました。東日本大震災・福島第一原子力発電所事故のあとの被災地で、住民の健康をどう守るかという課題に診療と研究の両面から取り組み、その成果を国内外の医学雑誌で発表してきました。診療をしながら研究を行った経験は、最新の医学的な根拠（エビデンス）にもとづいて診断・治療を考える姿勢につながっています。また、災害医療や高齢者医療の現場で「生活全体を見て健康を支える」ことの大切さを学んだ経験は、生活習慣病の管理や予防医療など、当院での診療にそのまま活かされています。

このページでは、院長のこれまでの研究・執筆・メディア活動をまとめています。災害医療や高齢者医療の現場で積み重ねてきた経験と、データにもとづいて物事を考える姿勢は、現在の西新井内科クリニックでの日々の診療にもそのまま活かされています。診療を受けていただくうえで、院長がどのような考え方で医療に向き合ってきたかを知っていただく材料になれば幸いです。

取り組んできた研究テーマ

院長の研究は、「災害や高齢化といった大きな変化のなかで、地域の人々の健康をどう守るか」という一貫したテーマでつながっています。患者さんの生活に身近な4つの領域に整理してご紹介します。

1. 災害後の健康影響

東日本大震災と福島第一原発事故のあと、地域住民の健康がどう変化したかを、死亡率・介護の必要度・避難の影響・救急医療へのかかりやすさといった観点から分析してきました。災害そのものによる直接の被害だけでなく、避難や生活環境の変化が、とくに高齢の方の健康に大きく影響することを明らかにしてきました。

2. 高齢者医療・地域医療

相馬市での診療経験をもとに、高齢化が進む地域での医療のあり方、ひとり暮らしの高齢者の孤立予防、介護需要の増加、地域コミュニティが果たす役割について研究・発信してきました。被災地の高齢者向け住宅「相馬井戸端長屋」での暮らしを通して、人とのつながりが健康を支えることを報告しています。

3. 生活習慣病予防・産業保健

糖尿病・脂質異常症・高血圧といった生活習慣病が、生活環境の変化でどう悪化するかを調べてきました。健康診断後の受診につなげる支援や、働く人の健康管理といった「病気になる前」の医療を重視しています。当院の生活習慣病の診療や健康診断にもこの考え方が反映されています。

4. 国際保健・医療へのアクセス

医療を受けにくい地域や国で、どうすれば必要な医療が届くようになるかという課題にも取り組んできました。症状から受診の必要性を判断する「症状チェッカー」など、医療へのアクセスを広げる仕組みについても論じています。

福島原発事故後の「間接的な」健康影響による超過死亡

相馬市・南相馬市の死亡データを震災の前後で比較し、災害による直接の死亡を除いても、震災直後の時期に死亡リスクが高まっていたことを示しました。とくに高齢の女性で影響が大きく、災害後の早い段階での健康面の支援が重要だと結論づけています。J Epidemiol Community Health. 2017. PubMedで見る

福島原発事故後の救急搬送時間への影響

原発事故のあと、相馬地域で救急車の搬送にかかる時間がどう変化したかを分析しました。事故後の約3か月間、搬送時間が大きく延びていたことを明らかにし、災害時に救急医療体制を維持することの重要性を示しました。BMJ Open. 2016. PubMedで見る

避難指示のあと、地域に残った住民の特徴

避難指示が出た地域で、どのような人がその場に残ったのかを人口データから分析しました。避難のあり方が高齢者の医療・介護に与える影響を検討しています。PLOS ONE. 2018. PubMedで見る

孤立を防ぐ高齢者住宅「相馬井戸端長屋」

震災後につくられた高齢者向け住宅での暮らしを通して、住まいと地域のつながりが、ひとり暮らしの高齢者の孤立や「孤独死」を防ぐうえで果たす役割を報告しました。BMJ Case Rep. 2022. PubMedで見る
